linearized. Question is: by setting e_UseBookmarks and linearized,
what are the effects in document download time? Is the document still
linearized?

A: The document will be linearized even with e_UseBookmarks.
Technically a PDF consumer (e.g. a viewer) does not need to download
pages referenced in the outline tree until they are requiested and the
response delay would be negliglible. Unfortunately there are all kind
of implementations out there and many are not optimal - also different
version of Acrobat viewer behave in different ways in this regard.
